Job description
The Health Families Team delivers a range of public health services to help improve health & wellbeing of children, young people and their families.
The post holder will be responsible for providing advice, support and delivery of public health interventions for the Healthy Families Service within one of the work streams. The post holder will work to support delivery of group, one to one support and screening programmes across multiple health improvement programmes for children, young people and their families. The post holder will be responsible for recording screening data, activity and outcomes to their Healthy Families Practitioner. One of the three posts will have a focus upon developing our digital offer.
To work as part of the Healthy Families Team to support various health improvement programmes which may include screening in schools for NCMP, Vision and Hearing, delivering group and one to one support across multiple health improvement programmes for children, young people and families in line with agreed pathways of care and within own competence.
