Healthcare Support Worker Band 2 – Transfer Team Full time – 12hr Shifts / Working Epsom General Hospital An exciting opportunity has arisen for Band 2 Healthcare support workers to be part of a newly created role within the Emergency Floor. The successful candidates will play an important role in supporting the nursing teams on both the Emergency Departments and the Acute Medical Units in facilitating a safe and timely discharge of patient from these areas. This role will help improve the patient flow across the Emergency Floor. The candidate will be expected to be able to work across both sites at Epsom and St Helier with days on each site to be assigned within the working rota We are looking for candidates who have excellent communication skills, team working skills and maintain the trust values we expect at all times Main responsibilities the post holder will have will be to support the Registered nurses to ensure that their patients are safe to transfer, this would include completing safe transfer checklist, record vital signs before transferring, give personal care if required to ensure patients are transferred in an acceptable manor and transfer the patient to the designated destination with assistance of the department portering teams Hours of work will be full time 37.5hrs a week, 12hr shifts from 1200-0000 with weekend and bank holiday working and working at both hospital sites Working under the direct supervision and direction of a Registered Nurse the post holder will work as the discharge team working across the Emergency Floor between the Emergency Departments, Acute Medical Units and other areas across the hospital. The post holder will take and receive handovers for patients from these areas and actively encourage patients to be transferred. Successful applicants will be responsible for the observations, patient care, nutrition and hydration, paperwork and transfer of patients from ED or AMU to other areas of the trust They will also work with the named nurse to ensure that patients are safely transferred by completing safe discharge checklist; property list checked and completed both in and out of the area.
