As a Sodexo Discharge Facilitator at Stoke Mandeville Hospital, you are motivated to make a difference in people's lives by providing support and planning to help our patients return to their homes after hospital stays, which can be a stressful period for some patients. What you’ll do:
- Liaise with the ward personnel and/or the hospital's discharge facilitator/team to identify clients that meet inclusion criteria and promote the timely discharge of patients who are clinically fit for release and meet agreed-upon criteria
- Implement non-clinical support that will help reduce the odds of discharged patients having future unplanned readmissions.
- To meet with prospective clients and their carers to explain the service, gain consent, and establish and implement their home care plan.
- Conduct a home visit for the client in combination with the agreed-upon discharge plan and OT/therapy evaluation to determine what pre-discharge activities have been requested to support the client's hospital discharge. E.G: adding a stair rail/building a wheelchair ramp
- Carry out basic housework / domestic duties
What you bring:
- Experience of working within the care industry
- Ability to communicate and interact in a professional manner with all stakeholders
- Excellent written and oral communication skills
- IT Literate
- Ability to prioritise effectively
- Planning and organisational skills
- Work with a substantial level of accountability

Package Description- Hours: 40
- Monday to Friday 9.00am -5.00pm with some weekend working when required which will be 10am - 4pm
- Rate of pay: £13.80
- Free parking

This position is only suitable for drivers with access to a vehicle (mileage is reimbursed at 45 pence per mile).
A DBS clearance check will be required for the successful applicants.
