This service will be able to respond quickly to support patients and their carers who are eligible for end of life care support in their own homes. It gives us the ability to offer one to one holistic care for people, supporting them at a time of their greatest need, in the place they have chosen to be, right up until the end of their life.
Working without close supervision, you'll have the opportunity to use your own initiative in line with the patient's established care plan. You'll be required to carry out a range of duties including monitoring symptoms, helping to assist with medications, undertake simple dressings and continence care, and keeping detailed records. You'll also help patients to meet their hygiene and dietary needs.
