An opportunity has arisen to work in an exciting integrated model for the delivery of patient care in Dorset County Hospital’s Acute Hospital at Home service. This service proactively manages and monitors a defined caseload of patients who have complex care needs in the short term, on a multi-disciplinary basis in their own home.

We are looking for a dynamic Healthcare Assistant/Therapy Assistant Band 2, to work in our multi-disciplinary team, ensuring a patient focused and individualised approach. Ideally you will have acute and community experience and have experience of effective multi-disciplinary team working in its widest sense. Car Driver and use of a car is essential for this post.

Main duties of the job

Our unique Acute Hospital at Home team are looking to recruit Healthcare Assistant/Therapy Assistants. Our service proactively manages and monitors a defined caseload of patients, who have complex care short term needs. The team is truly multidisciplinary, and consists of Therapists, Staff Nurses and HCA/Therapy Assistants.

Our close knit team are in the privileged position of caring for people in their own home, getting to know them well and seeing them recover. Our team and our service work with the patient’s needs at its heart and aims to support admission avoidance and early discharge. The focus of the service is the acutely medically ill and the elderly frail

We are looking for a therapist who:

  • Has 12 months experience in a healthcare setting
  • Has a special interest and passion for caring for the frail elderly patients of our community
  • Has evidence of community working

Patients are offered continuity of care by following them from hospital into the community and the post provides the opportunity to work more closely with community and primary care colleagues in a seamless service.

This is a service with the patient’s needs at its heart and aims to support admission avoidance and early discharge. The focus of the service is the frail elderly which is important in Dorset given that 27% of the population is of retirement age.

Detailed job description and main responsibilities

To ensure quality care and support service which enables patients to be supported throughout their care pathway whilst promoting independence and protecting choice, dignity, privacy and safety.

To work in support of all qualified staff in the assessment, planning, implementation and evaluation of care without direct supervision of a registered practitioner

To work as an associate practitioner in ensuring that holistic care is delivered, and standards of care are enhanced.

To communicate effectively at all times within the team, other health care professionals and with the patient family/carers.

Ensure high standards are maintained in a safe environment, which promotes equality and sensitivity for all individuals.

To regularly assess evaluate and report on actions and activities to registered staff.

To supervise and delegate work to junior members of the team.

Act as a resource and role model to others involved in the Acute Hospital at Home Service

Participate in the development, planning and delivery of the Acute Hospital at Home Service.
