We are looking to recruit a dynamic, motivated and enthusiastic Band 3 Healthcare Assistant who has a passion for delivering high quality care within a community setting.
The post holder will be working within the Haywards Heath Primary Care Networks (PCN) community team, based centrally in Haywards Heath, caring for patients within their own homes with the town of Haywards Heath and picturesque surrounding villages.
The friendly and professional team is made up of community nurses, health care assistants, clinical leads as well as coordinators, administrators, social workers, psychological therapist, mental health practitioner and Age UK community link workers, all working collaboratively to provide excellent care for our patients.

Community and healthcare experience is desirable, but not essential as training will be given to develop the skills and knowledge required for this role.
As a healthcare assistant within the community team, you will be visiting patients within their own homes to undertake a variety of clinical skills including: insulin administration, wound care, pressure area care, catheter care, observations, subcutaneous injections and venepuncture.

The post holder will need to be flexible and available to work across the hours of the service which is 08.00-18.00 7 days a week. Rosters are completed in advance.

- To work as part of a multidisciplinary team to provide health care, and support under the direction and supervision of registered staff to patients living in the community.
- Liaise with members of the multi professional team, to ensure care is delivered to the patient, maintaining effective communications.
- Undertake non-complex clinical procedures under the guidance of senior team including but not exhaustive to: venepuncture, sample collection, blood pressure monitoring, insulin administration, catheterisation pressure area care, wound care, and other tasks according to training and competencies.
- Empower patients to regain their health and independence in collaboration with other members of the Multi professional Team, liaising with clients, colleagues, family and other professionals in ensuring integrated quality care is provided, keeping accurate and timely records according to Trust policy.
