Job description
PLEASE SEE ATTACHED FULL JOB DESCRIPTION To provide end of life care to patients either within the specialist unit or within the community to meet the patient preferred place of care /death. To work as part of the multidisciplinary team to provide individualised care for patients and their families/carers. To have the flexibility to work across all aspects of the service to meet the needs of the patients, including in the community service and supporting the inpatient unit when required.
