Job description

We are looking for exceptional Band 3 Health Care Assistant to join us in our Brent Community Nursing Day Service. The service is run from 4.00pm to 12.00 Midnight including weekends. You will be working in one of the District Nursing team which covers the Brent wide area.

You should hold NVQ level 3 in health and social care and have a minimum of 12 months acute or community experience. If you are a motivated team player and want to excel in delivering exceptional holistic healthcare to a diverse patient group in their own home, we want to hear from you.

Previous experience of working in the NHS is essential, community experience is desirable, but training will be given. We endeavour to ensure you are given the best opportunities to excel and further your career to become Associate nurses and then Registered nurses within a 5-year plan.

CAR DRIVING IS ESSENTIAL.

As a Health Care assistant, you will be directly involved in facilitating and shaping self-care for patients with long term conditions in order to help keep them as well as possible in their own homes.

  • To contribute to the on-going assessment of a patient by completing the appropriate paperwork for allocated patients, recording and reporting all clinical and behavioural information to the nurse/therapist for action and planning purposes.
  • To undertake, and report back on delegated clinical activities that have previously been signed competent. E.g.: vital signs, routine urinalysis, glucose monitoring, gait/mobility training, falls prevention awareness groups, increasing independence through activities of daily living.
  • To enable patients/carers to make most effective use of resources within the service by ensuring they receive up-to-date information sheets, responding to any questions constructively.
  • To pass onto/seek assistance from other colleagues, promptly, any questions that you are unable to resolve.
  • To liaise and refer patients to the MDT team as appropriate in accordance with local procedures
