Summary of Role

Health and Wellbeing Coaches support people to take proactive steps to improve the way they manage their physical and mental health conditions, based on what matters to them. They support people to develop their knowledge, skills and confidence – or to build their “patient activation” - in managing their health and care, to improve their health outcomes and quality of life. Health coaches do this by coaching and motivating patients through multiple sessions to identify their needs, set goals, and supporting patients to achieve their personalised health and care plan objectives and providing interventions such as self-management education and peer support.

You will manage a caseload of patients, according to the needs, priorities and support required by individuals in the caseload. Your caseload will include patients with chronic diseases, such as pre-Diabetic (type 2 Diabetes), weight management, and those patients who have a reduced appetite for physical education. You will support, motivate, and encourage patients to make positive lifestyle changes, including diet and exercise, to help improve their health and wellbeing. You will also support initiatives to improve the health and wellbeing of local NHS staff.

Primary Responsibilities/ Clinical Practice

You will be working as part of the Teesdale Primary Care Network (PCN) team to support the patients of 6 GP practices (Barnard Castle Surgery, Old Forge Surgery, Gainford Surgery, Woodview Medical Practice, Pinfold Medical Practice and Evenwood Surgery) The PCN is recruiting a number of new roles to support the wellbeing of patients in this mixed urban and rural area, and you will be part of this proactive and expanding team.

You will work flexibly, supporting patients through one-to-one and group sessions delivered in GP practices, patient’s homes, and via online video consultations

Main Responsibilities;

Service Delivery

  • Provide one-to-one and group health coaching support for people with one or more long-term conditions, based on what is important to them, with the aim of: improving people’s levels of ‘activation’, empowering people to manage their own health and improve their health outcomes.
  • Manage and prioritise a caseload, in accordance with the needs, priorities and support required by individuals in the caseload. It is vital that you have a strong awareness and understanding of when it is appropriate or necessary to refer people back to other health professionals/agencies, when what the person needs is beyond the scope of the health and wellbeing coach role – e.g. when there is a mental health need requiring the patient be referred to an appropriately qualified practitioner.
  • Support network initiatives to improve staff wellbeing, including the Better Health at Work programme
  • Build and develop strong working relationships with health, social, and third sector providers in the Durham Dales area, including GP practices and the local Wellbeing for Life service
  • Work closely with professionals from a variety of health specialties, including GPs, practice nurses, practice pharmacists, and the wellbeing team including Social Prescribing Link Workers on an MDT basis
  • Build relationships with patients, patients’ families, and carers
  • Work flexibly, adapting to the needs of the service and client group
  • Collect service user feedback and and impact data, by using the Patient Activation Measurement, among other relevant tools

Communication

  • Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment, including motivational interviewing techniques
  • Give people time to tell their stories and focus on ‘what matters to me’;
  • Build trust and respect with the person, providing non-judgemental and non- discriminatory support, respecting diversity and lifestyle choices;
  • Encourage patients to make positive behaviour change
  • Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating

  • Maintain effective communication within the PCN and with external stakeholders, including by attending and contributing to practice and PCN meetings as required

Quality Management

  • Ensure compliance with any internal or external audits, including clinical, CQC and Health & Safety requirements
  • Ensure compliance with DDHF HR, performance and quality management systems and processes
  • Assist in identifying and coordinating the implementation of improvements to quality and performance management systems and processes
  • Contribute to the clinical governance process, including leading to adverse incident reporting/investigation as appropriate

  • Be willing to attend training with a non- clinical SSM health coaching skills programme (minimum 4 days) by a
  • Personalised Care Institute (PCI) accredited trainer or organisation
  • Coaching/counselling qualification/ experience or other relevant qualification/experience involving reflective listening skills relevant training and experience in non-clinical SSM Health Coaching through a PCI- accredited organisation

Knowledge/

Experience

  • Experience of using coaching approaches/frameworks and models or other helping strategies e.g. Motivational Interviewing
  • An understanding of the biopsychosocial model of health and the social determinants of health.
  • Experience of working in a multidisciplinary team
