Job description

Sunderland 0-19 Children's Service team warmly invite Qualified Health Visitors to join our friendly 0-19 team.

We are looking for a motivated, enthusiastic and responsible Health Visitor SCPHN to work within our 0-19 service. You will be passionate about improving the outcomes for children and families.

Your role will be to deliver the Healthy Child Programme 0-5 for families, planning, delivering and evaluating an efficient and cost effective service for an identified population. You will be delivering the National and Local Public Health agendas in response to identified health and social need. You will be delivering services as part of the wider Integrated team and thus will be working alongside our partner agencies in Children's services to deliver effective safeguarding of children
