Working with Newark & Sherwood Local Mental Health Team. An exciting opportunity has arisen to join the Newark & Sherwood LMHT. The LMHT provides specialised mental health assessments and evidence based treatment within Newark And Sherwood. The service is committed to the principles of recovery; aiming for people to be empowered to be as independent as possible and wellness recovery action planning. A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010. Newark & Sherwood Local Mental Health Team are looking for an enthusiastic, dynamic and motivated Band 4 Health Improvement Worker to work across the Newark & Sherwood area. This would be working with the LMHT working with secondary Mental Healthcare Services; you would work with patients identified as having serious mental illness and cardio-metabolic risk factors. People with a Serious Mental Illness die on average 15 – 20 years earlier than the general population often due to the under identification or poor management of their cardio-metabolic risk factors. This role will involve working alongside mental health teams and general practices to proactively identify and help the patient to manage these risk factors and will involve coordination and communication between the different parts of the service that the patients are in contact with. To carry out observations and take blood samples in order for risk factors to be identified. To support patients in understanding their risk factors, how to manage their risk factors and agree a plan with the patient for ongoing monitoring and management of the risks under the supervision of a registered clinician. The Health Improvement Worker will work as part of the Local Mental Health Team and wider PCN to deliver physical healthcare interventions to patients with Severe Mental Illness (SMI). The post holder will have a specific remit to support achievement of the physical health standards for people with SMI by engaging service users in routine physical health checks and support the person to work towards personal physical health goals in order to improve their physical and mental health. This involves supporting people on the GP Serious Mental Illness (SMI) register living in the locality to have their cardiometabolic risk factors measured. They will be 18+ so may be under the care of AMH, MHSOP or solely the GP.
