An exciting opportunity has arisen for a Band 3 Health care support worker to join the busy Neath community learning disability team. The post will work under the delegation of a Registered practitioner in the delivery of providing service users with the practical support and input required to ensure their health needs are met when accessing services, particularly aspects of primary care and elements of the annual health checks.
We are looking for a person who is able to organise their allocated case load whilst ensuring they are working within their competencies. They will need to have excellent communication skills to develop links and liaise with community services and specialist organisations.
The will need to demonstrate the values of the health board and be an active part of the community learning disability team.
The main aspects of the post will to be to work directly under qualified staff to support the physical health care elements of service users are being met and to liaise.

Working as part of a team, you will require a flexible approach, commitment to providing excellent standards of care, able to prioritise and organise your workload and communicate effectively within the team and the wider multi-disciplinary team.
Whilst based at Cimla Hospital you will be required to work across SBUHB sites when needed.
Work directly with service users to provide practical support to access Primary Care services including access to annual health check.
To provide information to service users on health and well-being.
Liaise with other services/agencies as appropriate.
In conjunction with Registered Nurse supervisor organises own caseload/workload including all administrative tasks and data entry.
To follow Health Board and Departmental Policies and Procedures in managing compliments, complaints and incidents.
