The City & Hackney Crisis Service and Home Treatment Team has an exciting job opportunity for 3 support worker ( 1 permanent and 6 months fixed term ) who is keen to venture into community based care or to gain some valuable experience in a Crisis Resolution and Home Treatment Team.
As part of the community transformation this is an amazing opportunity to gain experience and participate in development of the service. The post is aimed at an individual who is dynamic, recovery focused, client centred, flexible and innovative with highly developed skills in providing mental health care and excellent interpersonal skills.
The post-holders will work closely with individuals with serious mental illness (SMI), developing an understanding of complexity in order to provide the most appropriate support. You will be expected to be team player, self driven, have knowledge of different mental health illness and willing to work flexibly.
The hours you will be required flexible covering 24 hours a day, 7 days a week which includes night shifts and weekend on a rotation.
The post holder will be a member of a multi-disciplinary team providing support to clients and their carers when individuals are in a mental health crisis or in order to prevent admission to a mental health hospital or to facilitate early and safe discharge from the ward. The service covers the City the London and the borough of Hackney. The post holder will be expected to visit service users in their homes or in community locations. The role will entail providing personal, practical and social support to service users, their families and carers using the principles of recovery.
Hours of work will be over a 24 hour period, 7 days a week and on-call.
