An exciting opportunity has arisen to be part of an innovative team providing preventative services to individuals with moderate levels of frailty across South Staffordshire in the community. The post holder will be part of the Staying Well Service as a Health Care Assistant supporting the team in assessments and interventions around health promotion. health literacy and self-management.

We are looking for an experienced Band 3 Health Care Assistant who is enthusiastic to come and work in our multi-disciplinary team. The team is made up of Nurses, Mental Health Nurses, Occupational Therapists, Assistant Practitioners, Pharmacists and Community Connectors.

The service supports adults within their own home who have been identified as having mild to moderate frailty. The aim of intervention is to proactively manage the deterioration of frailty by taking a preventative approach. We support individuals to understand and manage their own health and wellbeing, promote healthy living and behaviours, link into local communities and reduce social isolation.

You will be working as a Band 3 Health Care Support Worker within our Staying Well Service Team, this will involve helping with organising and attending our Staying Well Hubs/Clinics in venues across South Staffordshire. Where appropriate you may be visiting patients within their own environment to carry out intervention and monitoring under the supervision of the qualified Nurses.

You will work in a person-centred way, working in partnership with the patient and others in order to support them in mangaging their own health and wellbeing. You will be an integral part of the Staying Well Service and work independently and autonomously where appropriate, and should be able to work alone as well as part of a team.

Key duties undertaken as part of the role include carrying out base line observations within the clinic environment and in patients' homes and delivering short follow up interventions as directed by the qualified nurses within the team.

To carry out the following specific clinical activities as requested by health care professionals within the Staying Well Team:

  • Measurement of blood pressure; pulse, blood oxygen levels and respirations.
  • Measurement of blood glucose levels.
  • To be aware of normal clinical parameters for each service user and to liaise with a registered nurse when the service user’s condition falls outside of these parameters.
  • To recognise a deteriorating or unwell adult and to liaise with a registered nurse and/or Staying Well Facilitator when necessary
All clinical activities to be documented clearly in the clinical notes and supervision provided by the qualified nurse/Staying Well Facilitator.

To contribute to the overall assessment, planning, implementation and review of individual service users under the supervision of qualified staff.

To be part of a person centred service where the needs of the individual will determine the type of intervention.

To help with the planning and the smooth running of the Staying Well Clinics/Hubs.

To work as part of a team and in conjunction with other professionals, statutory agencies and any associated service providers for the patient. To share updates and information regarding the service users that have been seen. Participating in bi-weekly team meetings and participate in service development meetings.
