Job Title: Health Care Assistant
Department: Inpatient Unit
Reports to: Ward Sister
Accountable to: Consultant Nurse/Lead Nurse Hampshire
Hours: 37.5 hours & 30 hours per week available, to be worked on a rota basis to include weekends, bank holidays, evenings/nights
Salary: Agenda for Change Band 2 - £20,270 - £21,318 per annum FTE
OVERVIEW OF ROLE
To support the trained nursing team in the provision of clinical care to the patients in the Inpatient Unit. To complete housekeeping and clerical tasks e.g. preparation of drinks, breakfasts, completion of menus etc. on a daily basis as required.
1. Responsibility for Patient and Client Care
- Promote a patient centred approach to care delivery by assisting patients in all daily living activities, as required by the nursing team and the patients care requirements.
- Ensure patient comfort, dignity and privacy at all times is maintained and report any concerns to the nursing team.
- Assist in the monitoring of patients’ blood pressure, temperature, pulse, respiratory rate, peak flow and oxygen saturation on completion of the internal training day and competency pack.
- Obtain specimens of body fluids as requested by the qualified nurse on completion of the internal training day and competency pack
- Assist in emergency situations, following appropriate training.
- Escorts patients to hospital and other appointments.
- Maintain high standards of care in an environment which promotes equality and sensitivity for all individuals
- Prepare patients drinks and serve meals as required
2. Communication
- Promote and demonstrate effective communication at all times as per Hospice values.
- Assist in the reception of patients, their relatives and general ward/department visitors
- Participate in ward team meetings
- On completion of the appropriate sections in the competency pack, document on the necessary paper work to record care provided (e.g. care plans, fluid charts, food and fluid charts, weight charts, stool charts.)
- Identify and use the most effective means of communication, appropriate to the situation and individuals involved.
- Manage own behaviour in a positive manner in order to maintain effective working relationships
- Observe complete confidentiality of all information including awareness and adherence to the legal requirements of the Data Protection Act.
- Respect the contribution of all team members
- Assist other clinical areas within the Organisation if the clinical situation or staffing levels require
3. Analysis and judgement will be necessary in the following situations
- Report any change in condition of patients to a qualified nurse
- Ensure any delegated responsibility you accept that you are appropriately trained to carry it out
- Maintain a safe environment for patients
- Participate in promoting and ensuring a safe and healthy working environment
- Be competent and practice safe infection control methods as per Hospice policy and competency pack completion. (e.g. safe waste disposal, universal precautions, isolation techniques
- Be aware and practice safe manual handling as per Hospice policy, following completion of the competency pack, involving training and assessment by an in house MAST trainer.
4. Tasks that require planning and organising
- Prepare the bed areas for admissions/transfers/procedures
- Order meals and liaise with kitchen
5. Responsibility for the development of policies and or services
- Participate in ward or departmental projects when required
- Set and maintain a high personal and professional standard
- Promote and engage in delivering a palliative rehabilitation and enablement approach
6. Responsibility for finance and use of resources
- Plan, organise, prioritise and control work, ensuring effective use of time, money resources.
7. Responsibility for managing staff and/or undertaking training
- Be willing to undertake training in NVQ Level 2/3 Direct care.
- Be willing to undertake training identified between you and ward manager in order to develop both personal and professional needs.
- Attend annual health and safety refresher courses, (e.g. fire/manual handling/blood borne viruses/resuscitation).
- Attend mandatory training sessions every 2 years, i.e. food hygiene/tissue viability.
- Participate in the orientation and induction of new staff.
- Link with a registered nurse to support a specific area of practice relevant to your working environment.
- To ensure training is received prior to using any equipment in the workplace.
8. This role will be required to use a range of information systems
- Be willing to undertake basic computer skills training and assist with delegated administrative and organisational tasks as agreed with ward sister
