An exciting opportunity has arisen to join the Telehealth team within Mersey Care NHS Foundation Trust as a Telehealth Health care assistant. The Telehealth hub is a busy and exciting Clinical hub service within the Trust. It supports patients with long-term health conditions and acute conditions to manage their condition using Technology at their own homes.

We are looking for enthusiastic and committed health care professionals with flexibility in working hours. The role involves working as part of a non-clinical and clinical team that provides support to patients.

This is an exceptional opportunity to be part of an expanding clinical team that uses Telehealth technology to prevent hospital admissions for patients with acute &chronic long-term conditions.

Working in partnership with Liverpool CCG and other health and social agencies, we have delivered this programme to over 12,000 patients so far.

The postholder will support the Telehealth clinical team and will be working in accordance with the hybrid working policy of the trust. You will need to have the ability to communicate with patients using telephony and IT /Excel/teams Offering telephone advice to patients and carers on a wide range of issues including use of equipment, health promotion, lifestyle advice, and self-care following training.

Principal Responsibilities:
Undertake a range of patient interventions over the phone as delegated by the team leader and other qualified clinical staff.
Offer advice to patients and carers on a wide range of health issues including health promotion, lifestyle advice self-care.
Plan and organise own work schedule once delegated from the team leader/clinical coordinator.
Work within existing clinical guidelines, policies, and procedures.
Offer advice and support to patients and families in a wide range of situations.
Communicate factual and sensitive information within confidentiality guidelines to patients, ensuring all information is appropriately documented and reported back to the team leader/coordinator.
Demonstrate own role to new starters and students.
Communicate information to a multi-disciplinary team and other agencies as appropriate.
Undertake ordering and receipt of stores.
Undertake further training to gain skills in other areas.
Document all patient interventions within electronic patient records.
Participate in PACE reviews to establish objectives, reporting arrangements, supervision and ongoing development.
Participate in mandatory training events.
Work with community services to improve patient participation and involvement.
The post holder shall, as necessary provide cover for and undertake duties of absent colleagues
