The community elderly virtual ward Team, deliver care closer to home. Interacting with the Consultant Interface Geriatrician, therapist, nursing and social care team

Required qualifications.

• Maths and English at GCSE C or above
• Level 2 diploma in health/ social care
• IT IS ESSENTIAL THAT YOUhold a full driving licence and have a car available for work on applying for this post

The post holder will provide support to qualified nursing staff in delivering care to patients in their own home in the community setting, enabling patients to recover at home following illness/ preventing the need for patients to be admitted to hospital.

We are looking for a health care assistant to work supporting qualified nursing staff overnight supporting patients in their own home
