Job Title: Family Support Worker
Contract: 22.5 hours per week (Monday - Friday working days and hours can be discussed) Permanent
Salary: £21,312.23 per annum (Pro rata £12,787.34)
Location: Dean Street, Bangor (other options possible)

Job Purpose:
As a Family Support Worker, you will offer practical help end emotional support to families and carers experiencing short or long-term difficulties.
Under the direction of the county manager, you will undertake support planning, provide one- to-one, group support and interventions to improve the lives of families and carers affected by serious mental illness. To equip families and carers with skills, knowledge and coping strategies to ensure their own well-being. This is to be achieved by visiting Carer’s within their own homes, in the community, at hospital settings or Carers groups established across the island of Anglesey, thus enabling Carers to manage the impact of their caring role.
The service also offers a time limited support service to the cared for, during which time the staff member will work with them to help identify their support needs, signpost them on to relevant services and offer some group work. In turn this will allow the carers to have some short respite opportunities.
Main duties & responsibilities:
- To have commitment to and demonstrate an undertaking of equality of opportunity and its relevance to different sectors within the community.
- Must demonstrate empathy towards Adferiad Recovery’s beneficiaries.
- Receiving and dealing with referrals from other agencies, as well as self-referrals.
- Providing services on a one-to-one casework basis.
- Providing services on a group basis.
- Identifying, recording and meeting the needs of individuals to enable them to maintain their wellbeing and caring roles.
- Providing information, advice and guidance as appropriate and in a timely manner.
- Assisting Carers in identifying areas of support and identifying the best option of support for them and/or the cared for.
- Providing emotional and practical support to carers and family members.
- Communicate clearly, effectively and professionally with individuals receiving service, Support Workers, Carers, Referrers and other Professionals via multiple formats (i.e. face-to-face, telephone, in writing etc.)
- Signposting/referring on to other specialist/appropriate services in a timely manner.
- Completion of individual support plans and exit plans.
- Producing reports for internal/external purposes
- Ensuring data collection and appropriate recording of records and presentation for monitoring, evaluating the effectiveness of the service and Quality Assurance reports as directed by the County Manager or other Adferiad Managers.
- To assist in collating carer feedback and information to evidence need for improvement of existing Carers services and/or development of new services.
- Follow procedures for general record keeping, data collection, saving and archiving – adhering to confidentiality and data protection policies.
- Actively seeking opportunities to promote the services offered by the organisation within the local community, and potential sources of referral. Including attending events for this purpose and the distribution of locally produced publicity material.
- To maintain an effective and responsive service to meet the developing and changing needs of the organisation.
- Act in accordance with the provisions of the Data Protection Act 1998 and Information.

Person Specification:
Essential Qualifications:
- There are no specific qualifications but there must be evidence of commitment to personal and career development relevant to the post.
- Holding, or a willingness to work towards a qualification in Social Care at Level 3 would provide evidence of competence for the post.
Essential Experience:
- Must be able to establish and maintain constructive relationships dealing with a wide range of people including Carers, family members, colleagues, other professionals and individual service users while maintaining appropriate personal boundaries.
- Must have proven experience of providing care and support services to people with a serious mental illness.
OR
- Proven experience of providing care and support services within a social care setting and or working with unpaid carers.
Essential Skills:
- Must be proficient in, and have well-developed IT skills – comfortable using Microsoft Office packages.
- Must have sufficient organisational skills.
- Must be able to ensure accurate and reliable administration of a service.
- Must have excellent communication skills and be able to communicate clearly in a number of formats.
