Family Carer Adviser (Carers Assessments)
£25,408 pro rata per annum (£22,661 for 33 hours per week)
Fixed term until 31 March 2026
Based in Claydon
An exciting opportunity has become available in our Adult Services team as a Family Carer Adviser (Carers Assessments).
This is a varied post, undertaking Carers Assessments and providing Carer Assessment processing support, and will report to an Adult Services Project Manager.
As part of this role, you will
- provide in-depth information, advice, guidance and emotional support to family carers in all communication formats as part of a carers assessment, using solution focused, health coaching and signs of safety approaches.
- To work to set targets to undertake Carers Assessments
- Process the Dementia Project referrals of the Carers Assessments
- Support family carers with practical help
· work collaboratively with the Carers Assessment Co-ordinator and Project Manager to review systems and procedures that promote efficiency and effectiveness of the service with regard to Carers Assessments, and undertake administration tasks to assist this process.

· Understanding of voluntary and statutory services within Health and Social Care
· Be able to work autonomously and work with the project manager to develop service and need of family carers
· Driving Licence Preferred
This role will be subject to an enhanced DBS check and suitable references, which will be carried out following acceptance of any offer made to the successful candidate
