We are currently recruiting for an experienced Care Coordinator to join a hard working and dynamic team.
As a Care Coordinator your role would include but not be limited to the following:
- Scheduling weekly care visits for our community based staff
- Identifying capacity, bidding on and scheduling new care packages
- Contributing to our On-call / out of hours service
- Providing excellent levels of customer service
- Adapting to changes and dealing with pressure in a sometimes fast paced environment
- Liaising as necessary with service users, next of kin, professional partners
- Working alongside our Quality Monitoring Officer to arrange new service user assessments and care plan reviews as required
- Ensuring a high-quality and prompt service at all times
In reward for your hard work you will receive a salary of £20,000 to £21,500 and generous bonus scheme for achieving pre-set targets.
