PCN Digital and Transformation Lead

Purpose of the role
This is an excellent opportunity to be central in developing digital solutions in a large and rapidly- growing PCN. This is a senior management role with the post-holder working closely with the leadership team and ICB digital transformation team to create a vision to guide service improvements, identify opportunities to make tangible positive transformation, and provide the inspiration, expertise and leadership to deliver changes in a collaborative way.

The post-holder will help the PCN to adopt new technology and other initiatives to improve access to care and enable PCN staff to work more effectively to support the sustainability of general practice services. The post-holder will work on a range of digital transformation projects and is expected to be an experienced project manager and transformation lead with considerable project experience, ideally within an NHS environment.

Main tasks and duties
The post-holder will:

  • Detect and assess emerging technology / systems relevant to Primary Care whilst recognising potential impact/pitfalls.
  • Improve adoption of new technology to enhance patient access and experience, increasing Ely

PCNs productivity.

  • Be responsible for a range of digital transformation initiatives for Ely PCNs, managing priorities and inter-dependencies.
  • Ensure that digital and operational transformation is embedded and aligned with system and national strategies, working closely with the ICB Digital Team.
  • Build relationships and facilitate collaboration between practices within the network to support the delivery of care to patients (e.g., delivery of enhanced access or other hub-type working arrangements).
  • Review and improve where required Ely PCNs digital maturity, identifying opportunities for digital optimisation within the PCN.
  • Support the effective adoption of national and local initiatives, including integrated working at neighbourhood and place level to improve access to patient services.
  • Support the leadership team and practices with understanding and adhering to the General Data

Protection Regulation (GDPR)

  • Use data to: o Understand demand, capacity, and activity - helping to drive improvements in patient experience of access o Support Population Health Management, improvements in care quality and PCN operational efficiency o Understand the type and intensity of support needs of the PCN
  • Drive the planning and prioritisation of changes that will improve the delivery of care and improve patient and staff experience in the practices within the PCN.
  • Develop and manage an overall transformation plan for Ely PCNs, including managing risk, considering the potential impacts on the organisation, and determining resource requirements.
  • Provide feedback to PCN leadership team and member practices on the progress of current initiatives, capacity for change, improvement, and issues so that plans can be adapted in response.

Communication and Engagement
With responsibility for developing and supporting the delivery of transformational projects across Ely
PCNs, the post holder will require expertise in communications and engagement, specifically in bringing people together and gaining support and buy-in to change.

This expertise will be used to:

  • Build and maintain good working relationships with a broad range of internal and external stakeholders.
  • Initiate and participate in the development of effective systems of communication and cooperation between member practices, staff, and other primary care team members.
  • Ensure that changes are aligned with agreed clinical priorities and have ownership and engagement from clinical leaders and staff.
  • Support practices to communicate changes and new processes effectively and consistently to patients.
  • Identify best practices and learning from other areas that could apply to the Ely PCNs and communicate this to staff.
  • Make connections across the PCN to support learning between practices and sharing of best practices.
  • Connect with commissioners and other service delivery organisations to plan and deliver changes that will improve working between general practices and other primary and non-primary care services.
  • Lead or participate in relevant working groups, project groups, service meetings and other forums across the PCN to agree on changes, share learning, discuss challenges, and resolves issues.
  • Represent the PCN in external groups or bodies as required, including in potentially sensitive situations.
  • Consider accessibility requirements of patients (language, culture, literacy, hearing for example).

Information and Data Management
The post holder must understand and use data to drive improvement and assess impacts.

These skills will be used to:

  • Take a data-driven approach, looking at evidence and information available to support understanding of challenges, identify possible solutions and track the impacts of changes that have been made.
  • Bring together data from various sources and use information to develop insights into working practices, effectively tailoring content to meet the audience's needs.
  • Effectively present and communicate data to practices and PCN staff at all levels to increase understanding of operational challenges.
  • Interpret data and highlight variation to support open discussion about where variation is and is not warranted.
  • Help Ely PCNs and practice staff understand the limitations of particular data sets and where other information may be available to supplement understanding of issues.
  • Support and encourage a focus on data in understanding the effectiveness of changes made.
  • Seek and secure analytical resources to support Ely PCNs in developing and implementing appropriate data and information-sharing mechanisms.
  • Use data to track and assess the impact of change.

General responsibilities

  • Maintain and monitor high standards of care and work flexibly when required in the interests of the practice and patients.
  • Maintain accurate, comprehensive electronic records of all consultations and treatment, using

SystmOne clinical system.

  • Demonstrate understanding of the implications of national priorities for the PCN and member

Practices.

  • Commitment to ensure all Health and Safety requirements and Infection Control measures are met and to report any problems to the PCN Practice Manager.
  • Commitment to the effective use of NHS resources, avoiding unnecessary waste.
  • Demonstrate an awareness of own limitations and experience, and proactively seek solutions and learning opportunities accordingly.
  • Participate in data collection, audits, and performance achievement targets as required.
  • Coordinate annual leave and absence with colleagues to ensure the continued delivery of a high quality service to our patients and colleagues.

Learning, training and development

  • It is the responsibility of the employee to comply with all organisational and statutory requirements (e.g. health and safety, infection control, equality and diversity, confidentiality, safeguarding adults and children, information governance).
  • Complete all mandatory and statutory training required by the role.
  • Take responsibility for personal development, learning and performance and maintain education through attendance on any courses and/or study days necessary to ensure that professional development requirements are met.
  • Identify personal development and training needs in conjunction with the PCN Manager and

Clinical Directors and actively engage with annual appraisal and ongoing CPD portfolio.

  • Engage in annual appraisal, developing objectives to inform a Personal Development Plan, which may include 360-degree appraisal and use of patient feedback.
  • Support the practice staff and respond to requests for advice and assistance.
  • Undertake additional training where necessary to provide enhanced services and participate in training programmes implemented by the PCN/practices as part of this employment.
  • Actively engage with any learning following complaints, untoward incidents and significant event audits.

Interpersonal

  • Ensure clothing, personal grooming, and manner are at all times professional, safe, infection- control suitable, and are not likely to hinder relationships with colleagues or patients.

Safeguarding
The post-holder will have responsibility to make all reasonable efforts to safeguard adults at risk and children, and promote their welfare, by following the practice policy, keeping up to date with training and learning and communicating issues of concern.

Patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately.

In the performance of the duties outlined in this job description, the post-holder will have access to confidential information relating to patients and their carers, practice staff and other healthcare workers. They may also have access to information relating to the PCN and member Practices as business organisations. All such information, from any source, is to be regarded as strictly confidential.

Information relating to patients, carers, colleagues, other healthcare workers or the business of the practice may only be divulged to authorised persons in accordance with the practice policies and procedures relating to confidentiality and the protection of personal and sensitive data.

Health and Safety
The post-holder will take responsibility for their own health and safety and wellbeing, and take reasonable steps to ensure the same for other persons, including acts of omission. Key ways to ensure health and safety obligations are met are being familiar with the practice health and safety policy, ensuring mandatory training is completed annually, and communicating any concerns to the named practice health and safety lead in a timely manner.

The post-holder will follow infection control policy and guidelines for safe practice, again communicating any concerns in a timely manner.

About Ely Primary Care Networks
Primary Care Networks formed in 2019 with the aim of supporting GP Practices and driving improvements in population health and wellbeing at a local level.

Ely North PCN and Ely South PCN are the two main PCNs in the East Cambridgeshire region, linking six of the eight GP Practices in East Cambs. Our two PCNs work closely with one another and have a joint
Board. Together we are known as “Ely Primary Care Networks” and support over 75,500 patients.

  • St Mary’s Surgery, Ely Haddenham Surgery, Haddenham
  • St George’s Medical Centre, Littleport Staploe Medical Centre, Soham

Ely PCNs employ over 30 fte staff including Social Prescribers; First Contact Physios; Care Coordinators; Health and Wellbeing Coaches; a Substance Misuse Worker; Pharmacy Technicians; and Clinical Pharmacists. Ely PCNs is supported by full-time managers, a Lead Nurse, GP Leads for both diabetes and frailty, two forward-thinking Clinical Directors, and a supportive Board.

PCNs are now established as key System Partners and looked to for the development of health and social care at neighbourhood level. Ely PCNs are well-respected and have excellent relationships with local partner organisations and the Integrated Care Board. We are known as a PCN that can be innovative and deliver successfully thanks to projects such as our infant feeding support for new parents, rapid-response substance misuse worker, neighbourhood-level winter wellness project, menopause support, and a superlative local Covid vaccination programme.
