An exciting opportunity has arisen to join the team on Pierce ward as a B54 Discharge Facilitator.
This role will involve working within the Multi-disciplinary teams to assist in the patient discharge process, improve the safety of discharges and ensure there are minimal delays.
A candidate should be IT literate with particular regard to hospital IT systems and have excellent communication skills to enable discussions with the MDT and community services.
- To provide support in order to facilitate and drive the safe, effective and timelytransfer of patient care from the Trust to the most appropriate setting to meet theneeds of the patient
- To promote the philosophy of discharge planning as an integrated whole system approach which delivers an effective service
- To take active steps to reduce the number of patients whose further care is delayed
- Actively communicate with patients and their carers with the continuous process of discharge planning and ensure they are involved at every stage
- Be responsible for the co-ordination of patient discharge and to support thedevelopment, implementation and evaluation of such processes
- Liaise with Registered Nurses, Doctors, Therapy Services, and Social Services, CHC team, Housing and any other appropriate members of the Multidisciplinary team to provide a proactive and continuous process of discharge planning
