Job description

We are looking for a Discharge Co-ordination Unit (DCU) Senior Officer to join the Barking & Dagenham, Havering and Redbridge Integrated Discharge Hub (BHR IDH). The service seeks to support the BHR system to provide the best opportunity for people to maximise their independence by co-ordinating a safe and timely discharge to the right destination, with the right information at the right time, with the right onward care, resulting in a positive discharge experience.

The BHR IDH team comprises of enhanced Physiotherapy & Occupational Therapy support, Discharge Coordination Officers, Rehabilitation Assistants and Admin Staff support, who cover the 7 days service to ensure safe and timely hospital discharge of patients.

You should have a passion for delivering a high-quality service and be dedicated to driving changes which improve the experience of the care and services the Trust provides for patients and service users.

The BHR IDH is the main point of contact for the discharge of patients that require health, social care and community services. The team works closely with London boroughs of Havering, Barking & Dagenham and Redbridge Adult Social Care teams to this means we are able to facilitate safe and timely discharge of patients via the Trusted Assessor pathway.

It is paramount that discharge flow of patients is maintained throughout the Trust, and the DCU Senior Officer will be a key facilitator of this. They will ensure all referrals are processed within agreed timeframes, that any barriers to discharge flow are identified at the earliest point, and liaise with all relevant partners to ensure minimal impact and reduce discharge delays.

The post holder will be responsible for the day to day operation of the service, playing a key role in building internal and external interfaces, liaise with partner organisation for safe and timely processing of discharge referrals and provide direct line management support to DCU Officers, ensuring strict compliance with Trust policies and procedures.

You will oversee activity and performance recording by ensuring high standards of record keeping; that all information is timely, accurate and meets all Key Performance Indicators (KPI) reporting requirements. The successful candidate will have excellent communication skills and have experience of dealing with difficult or challenging situations or individuals.

The post holder will report to the DCU manager.

This is a full time post working 37.5 hours per week covering core office hours of 09:00am to 5:00pm.

Probationary Period

This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process, new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

High Cost Area Supplement - London

This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (minimum payment of £4,108 and a maximum payment of £5,177).
