An exciting opportunity has arisen to join our dynamic dietetic team within our Pulmonary Rehabilitation service in Swansea Bay.
You will join a well established multi-disciplinary team serving our local population of chronic respiratory disease patients and also provide assistance in delivering rehabilitation to our long covid patients.
Our services are based out in the community, a car is essential. You will be based in Neath Port Talbot but may be required to travel to our other localities in Swansea and Bridgend if needed.
- Following clinical protocols to manage own caseloads of Clients in community settings including the clients’ own homes carrying out initial interviews and providing appropriate advice within established guidelines, this could be with other members of the Pulmonary Rehabilitation team or lone working as appropriate.
- Assessing client progress and within the parameters set by the dietitian to make relevant alterations to existing ACBS prescription or move a client’s dietetic plan onto a different stage within agreed protocol based clinical care. This could be done without prior consultation with dietitian and may include discontinuing ACBS products if appropriate.
- Enabling, supporting, supervising and assisting Clients who are undertaking individual treatment programmes, which have been agreed with and planned by the Dietitian. This will be carried out community settings including the clients’ own homes
- Reporting and record all client observations, information and outcomes in line with service standards in a timely manner
- Development of Dietetic Resources, monitoring stock and ordering appropriate supplies from manufactures.
- Delivering education to groups of patients, carers and Health Care Professionals
- Assisting in data collection and production of indicators of outcomes and group education audit reports.

- To follow clinical protocols manage own caseloads of Clients
- To undertake specific duties, without the Dietitian present which will include:
- Carrying out initial interview and provision of appropriate food first advice.
- Encouraging and monitoring clients to follow dietetic care plan
- Carrying out nutrition risk assessment including weight, height BMI and percentage weight loss.
- Calculation of nutrition requirements and dietary analysis
- To enable, encourage, support, supervise and assist Clients who have had a treatment programme formulated by the dietitian. These Clients will have a range of respiratory medical conditions and following clinical protocols (working under the direction of the dietitian)
- To deliver of structured education programmes to groups of patients or health care professionals
- To audit stock levels of ACBS products and supporting care homes to manage stock control efficiently and effectively
