Job description
Provide a high quality one-stop shop diabetes service to patients in the community setting by delivering all 9 annual healthcare checks to patients with diabetes, incorporating phlebotomy, blood pressure monitoring, BMI, foot screening, eye assessment, urine testing and smoking advice. Work independently to perform annual diabetic foot screening assessments and use own clinical judgement to refer into podiatry services where required. Use Doppler scanning/palpation techniques and monofilament testing to identify potential peripheral vascular disease. Give advice and guidance to support good foot health.
Provide support to the retinal imaging aspect of the service by capturing and assessing high quality digital photographs and managing own clinical caseload to ensure all images taken meet the national requirement for quality and grading. Ensure that images which do not meet the required standards are retaken or referred for further assessment and that all service users are given appropriate advice on individual clinical outcomes. Provide a safe, effective and efficient blood taking (phlebotomy) service for the purpose of diabetic screening. Ensure height and weight measurements are taken and a BMI calculation recorded for all patients.
Ensure that support and advice is offered with regards to smoking cessation and services which patients can access for further information. Facilitate the collection of a urine sample from each patient and prepare this for transport to the laboratory Measure and record accurate blood pressure readings and record within appropriate software. Perform pre-screening assessments on patients attending for retinal screening by administering eye drops and recording visual acuity measurements. Deal with members of the public and professionals in a courteous and professional manner.
Deal with patients sensitively and respectfully for what can sometimes be a distressing or anxious experience. Give advice and instruction re screening procedures to patients, relatives and other healthcare professionals. Receive and deal with daily communications from a variety of sources (telephone, e-mail, paper documentation) maintaining confidentiality at all times Maintain and support efficient data collection systems ie EMIS, ICE. Apply standard principles of infection control to all activities at all times Participate in audits as required by the service.
Keep up to date by training and identify any training needs on annual PDP. Display a positive and considerate approach to patients and their families/carers. Responsible for the health and safety of themselves and others and for reporting and actively co-operating in the application of the Trusts health and safety policies and procedures.
