DEMENTIA AND CRISIS TEAM SUPPORT WORKER Band 4

We have vacancies in this exciting service set up within Bedfordshire where you will have the opportunity to work with people living with dementia and other mental health diagnosis in order to reduce distress and increase their quality of life as well as those that care for them.
The Dementia Intensive Support Service and Older People’s Crisis Team will provide an urgent response to people living with dementia and other mental health conditions in their own homes and 24hour care settings 7 days per week, 365 days per year from the hours of 9am to 8pm. With a 4 hour response time, timely and effective assessment and excellent care planning strategies will be key to helping people to stay in their own homes with reduced levels of distress, reduce 1:1 supervision and prevent unnecessary hospital admissions and transfers of care.
If you are passionate about Older People living with Dementia or other Mental Health conditions and are skilled in providing person centred care, we want to hear from you. We are looking for applicants that have the necessary skills and experience and may come from either in patient or community background.
