Job description

The main purpose of this post it to support the Clinical Nurse Specialist (CNS) and the breast team in coordinating patients’ care with a diagnosis or suspected diagnosis of breast cancer during the diagnostics, treatment and surveillance.

This job is a great opportunity to anyone who is willing to progress and to take on new challenges whilst working alongside an expanding breast team in Buckinghamshire NHS Trust.

Main responsibilities:

• To manage and co-ordinate patients on a Personalised Stratified Pathway
• To have responsibility for adding and monitoring patients who are managed on a remote monitoring system (Infoflex)
• To act as a point of contact for patients that are self-managing their follow up and escalating symptoms to the Advanced Nurse Practitioner(ANP)/Physician Associate (PA)/nurse specialists or Consultants
• To work closely with the nursing and non-nursing team under direct supervision
• To capture patients’ activity on relevant hospital electronic systems
• Set up Health Needs Assessments as instructed by a nurse specialist/ANP/PA
