You will join Aneurin Bevan University Health Board as a Consultant Geriatrician with an interest in Intermediate Care during an exciting period of transformation for our services.
This post provides you with an opportunity to be the clinical lead with responsibility for driving forward transformation and service redesign in relation to the provision of appropriate complex care at home for older people across Blaenau Gwent, where step up is required, you will clinically lead the 8 bedded step up direct admission beds located at Ysbyty Aneurin Beavan in Ebbw Vale.
As a key member of the Community Resource Team, the Consultant is responsible for leading and developing new rapid response and direct admission pathways to ensure that those who require complex care are able to receive it in a timely manner and, for ensuring that the highest standards of complex care are delivered to patients in their own homes.
The team has a positive reputation as a service providing a consultant led dynamic , responsive and high quality person centred complex care at home service We have outstanding integrated services which benefit from speciality Doctors, ANP Nurses, Prescribing nurses, Pharmacist, Respiratory Specialist, Physician’s Associate, Occupational Therapists, Physiotherapists, Assistant Practitioners and Support Workers who constitute our rapid response, discharge to assess, falls and reablement teams.
Demand for our services are growing, you join us at a time of change using key information from the framework for Older People, Strategic Programme for Primary Care, Integrated Care for Older People, place based care and the Older Persons Commissioner’s report – Leave no one behind as our guiding principles to re-design and develop a whole system approach to delivering services for older people in Blaenau Gwent.
You will be a key driver in developing a model that works collectively across organisations to provides both clinical and non clinical interventions for frail, elderly people so that they can stay at, or as close to home as it is safe for them to do so.
