This post has a responsibility for the provision to be a part of a nursing and care team providing 24- hour care to a patient with complex care needs in their own home. This role is for exceptional nurses who have a passion for life and a genuine desire to achieve the best and improve the patient’s quality of life.
DUTIES AND RESPONSIBILITIES:
To work within and uphold the company’s workplace values at all times, which includes providing a person-centred care and commitment to quality care support and well-being
To provide personal care adhering to privacy and promoting dignity and working in collaboration with the service user and other members of the health and care team
To support the patient with communication needs to meet complex communication requirements
To complete nursing assessments as needed and as appropriately as possible
To take appropriate action in emergencies and unexpected events
To maintain confidentiality at all times
To communicate effectively with the wider team, MDT and other health professionals to ensure continuity and quality of care
To actively participate in the supervision and appraisal system. Will need to be proactive in identifying any learning requirements and opportunities
To adhere to the NMC Code of Professional Conduct at all times, maintaining NMC registration and will be responsible for maintaining registration portfolio.
CLINICAL DUTIES AND RESPONSIBILITIES:
To provide clinical intervention to aid with patient’s daily nursing requirements and needs such as:
- Tracheostomy
- PEG (Percutaneous Endoscopic Gastrostomy)
- Ventilation Management
- Oral Suctioning
To provide routine and emergency tracheostomy care including nebulisers, suction, emergency decannulation care, etc, in accordance with client care plans
To administer/assist with medication via PEG
Keeping accurate MAR charts records
To monitor and respond to clients complex health needs. The level of support can vary from day to day dependant on the clients physical and mental health and arousal levels.
Ensuring the correct level of infection control practice are adhered to when undertaking clinical procedures and at all other times throughout the day.
Make clinical decisions throughout your shift, based on specialist knowledge of the client’s individual needs and care plan protocols, and responding to any change with the client’s condition
- Ensure open communication with the Clinical Lead Nurse and report any concerns or issues without delay

- Overseas applicants : Please note that to qualify for a skilled worker sponsored visa you MUST reach the following requirements; , you need to be a qualified nurse, doctor or social worker in any country or have a NVQ level 3 (or equivalent) in health and social care and you MUST hold a valid driving license.
