The post of Support Worker Health Care Assistant (HCA) will involve working in Neighbourhood teams and the new Neighbourhood Recovery Service (formerly CMHT). This will involve contributing to a developing service through a period of considerable change.

An opportunity has arisen for a Community Support Worker for Physical Health, to join a vibrant integrated Community Mental Health Service in Hackney. The successful candidate will focus on the physical health and wellbeing of our service users with mental health needs and should understand the relationship between physical and mental wellbeing.
The successful applicant should have existing skills to undertake physiological assessment of our service users with Severe and Enduring Mental Illness. They will take and record ECGs, blood pressure, pulse, weight, BMI and venepuncture for required blood tests, as well as assess and advise on healthier lifestyle changes (exercise and activities, eating and drinking, smoking, alcohol and drug use) and signpost to other services.
The post holder will work alongside Community Mental Health Services under the supervision of the team manager, running physical health clinics, undertaking physical health checks and general health and wellbeing assessments. This role is vital in enabling service users to access the support they need to recover from their mental health and physical health problems, promoting recovery in order to return to everyday living.
The successful applicant will work alongside the mental health team in clinics and home visits, and will work autonomously, managing their own diary.
