Job description
This role will be part of a multi- disciplinary team based within the Forensic Outreach and Liaison Service which is part of our Trusts commitment to deliver excellent and responsive assessment, treatment and care for people whom experience mental illness and present with high risks to the public. This is an integrated role working closely with the local authority and other stake holders such as probation, Ministry of justice and the criminal justice system.
You will provide time limited 1:1 support to clients giving information and/or support to allow them to access a wide range of agencies and services that can provide help and advice with for example, housing, social isolation and exclusion, education, debt management and employment problems. The post holder will support people who are typically hard to engage, to access and utilise community services in order to improve their use of resources, which is likely to improve their mental health, social difficulties and reduce the risk of them reoffending.
The post holder will actively engage and work with a small number of individuals with offending histories with a wide range of mental health problems who have a level of co-existing complex needs including homelessness, substance misuse, significant risk and social exclusion and who at times may present with extremely challenging behaviours.
- Supporting the service with developing an assertive outreach approach when engaging with clients in the community.
- Offer support to our clients when engaging in community activities and education.
- Providing increased support to our clients when transitioning back into the community.
- Undertaking low level mental state examinations in the community/hospital.
- Becoming involved in a sub-group that will focus on moving clients through different levels of hospital security under supervision of qualified staff.
- Impart highly complex and sensitive information to: people using the service and their families/carers re assessment, care planning, treatment and review.
- Contribute to the team’s engagement with a wide range of stakeholders including staff, people using the service, their carers, GP’s, probation, MOJ criminal justice and other statutory and voluntary sector agencies.
- Liaise with a wide range of other professionals and agencies to ensure the provision of consistent well-coordinated care; secondary mental health multi-disciplinary teams, Primary care teams as well as external agencies also involved in the provision of care e.g. voluntary and private sector, police, social services, housing, employment etc.
We actively seek to engage people who use our services and our communities in improving the mental wellbeing of the local population. We work closely with other NHS and voluntary sector organisations who provide services and support people who use services and carers.

For international travel, both Gatwick and Heathrow airports are nearby.
