Job description
The team is based at Hall Court in Telford and provides mental health assessment, treatment and support to people in Shropshire. As an integral team in the Urgent Care Pathway we work 24 hours a day, 7 days a week. We work with people who are experiencing a mental health crisis, require extra support alongside their regular support network or who can have the same treatment provided at home that they would receive in a Mental Health Hospital.
We are a team that consists of Band 5 and 6 Nurses, Band 3 Support Workers, Non-Medical Prescribers, Medical Staff, Occupational Therapists, Administrators and Business Support Staff. As we work with people with all types of mental health presentations, we need to have a range of skills and interests that create a strong and responsive team.
We have a vacancy for a Band 3 Community Support Worker on an 18 month fixed term contract to cover maternity leave.
Mental Health Practitioner – Nurse, OT, Social Workers
Working in a multi-disciplinary team and in partnership with internal and external agencies, you will;
- Provide specialist assessment and care planning in conjunction with service users and carers
- Evaluate treatment plans and undertake home treatment
- Triage and assess referrals received by the service in liaison with multi-disciplinary colleagues
- Co-ordinate the work of the Team
Working in a multi-disciplinary team and in partnership with internal and external agencies, you will;
- To provide support, and spend therapeutic time with an allocated group of service users with mental health needs.
- To promote service-users recovery and to assist them to maintain their place in the community or manage transitions between services.
- To assist the Shift co-ordinator/ Lead Professional to plan, implement and evaluate individualised care plans.

- Communicate with service-users and carers, all members of the multi-disciplinary team and staff in external agencies using a variety of methods including Information Technology.
- Reports and effectively communicate service-users health status and care aims in care records and verbal reports to care-co-ordinators and other members of the multi-disciplinary team.
- Relate appropriately with service-users, carers, colleagues and others – at all times maintaining professional boundaries.
- Works in partnership with service-users to assist them in identifying activities that are meaningful to them and that support agreed health outcomes.
- Regularly supports service-users to access a range of activities and services to maintain their independence and promote their recovery.
- Regularly provides advice and guidance – promoting healthy activities and lifestyle choices.
