Job description
Duties:
- Link and coordinate appointments and link client with transportation if needed to appointments, activities, and community outings.
- Implement behavior management strategies to promote positive behaviors
- Assist with demonstrating and encouraging to Maintain and clean and organized living environment
- Assist and encourage with mental health, Physical and other appointments concerning health.
- Assisting and identifying support systems for consumer.
- Assisting with crises.
- Assisting with locating stable housing and encouraging the consumer to maintain stable housing
Qualifications:
- Ability to drive and possess a valid driver's license
- Knowledge of resident rights and ability to advocate for them
- Strong behavior management skills
- Ability to prepare meals according to dietary guidelines
- Excellent organizational and cleanliness skills
