Wolverhampton Community Intermediate Care Service is expanding its successful community therapy team to provide an enhanced response to hospital discharges and admission avoidance between 8am and 8pm (7 day working pattern). We are looking for a dynamic band 6 physiotherapist who has excellent communication and prioritisation skills with a flexible person-centred approach to join our supportive team.
This exciting opportunity involves working across the hospital to community interface, including the option of accessing a wide range of rotations supporting professional development.
Our community team includes Physiotherapists, Occupational Therapists, Rehabilitation Assistants, Senior support workers and Therapy Technicians. The service covers the Community Intermediate Care Team (CICT), Rapid Intervention Therapy (RIT) and Community OT and Physiotherapy.
RIT therapy enables same day access to therapy to prevent hospital admission and involves joint working with urgent care advanced clinical practitioners, nurses, and paramedics with access to consultant support.
CICT provides short term (up to six weeks) rehabilitation and re-enablement programmes to patients in their own homes following hospital discharge, illness or injury.
This role involves working extended hours - 7-day working pattern.
A full driving licence and access to a vehicle that can be used for work is essential.
To complete physiotherapy assessments to in the community preventing admission to hospital, and providing community rehabilitation. This will include treatment planning, assessing risk, promoting and enabling independent functioning.
To provide training, supervision and advice to students, band 5 therapists, rehabilitation assistants and therapy technicians.
To participate in the planning, development and evaluation of community therapy service, holding responsibility for defined projects.
To maintain close working relationships with other health professionals /referrers to ensure efficient, effective and appropriate delivery of the service.

To accept clinical responsibility for your own caseload and to prioritise patients based on clinical need, making effective and efficient use of time available and support more junior staff to do likewise.
To undertake comprehensive physiotherapy assessment of patients with a range of pathologies using clinical reasoning skills and manual techniques to provide accurate analysis of needs
To undertake home visits usually as a lone practitioner or with other members of the team to assess and review patients.
To plan, evaluate and update individualised physiotherapy treatment based on a sound knowledge of evidence based practice and treatment options.
To communicate effectively with patients and carers to ensure understanding of their condition in order to maximise rehabilitation and to minimise effects of complications.
To provide advice and instruction to relatives, carers and other professionals to promote understanding of the aims of physiotherapy and to ensure a consistent approach to patient care.
To provide physiotherapy specific teaching, training and advice to students, colleagues and members of the Multi- disciplinary team in the management of patients.
To educate, supervise and manage allocated staff. This will include informal discussion, joint working sessions, clinical supervision as well as formal appraisal and personal development plans.
To take an active role in the MDT to ensure delivery of a co-ordinated patient treatment plan. This includes daily liaison over patient’s progress, attending weekly meetings, writing letters and reports to identify patient’s management, progress and comprehensive discharge plans.
To possess risk assessment skills of community situations e.g. visits posing a risk to personal safety, demonstrating the ability to act appropriately to ensure your safety and that of others when functioning as lone practitioners.
To identify when it is appropriate to refer to other agencies e.g. social services and to make referrals to other disciplines as required.
To develop and maintain skills required to practice through continual professional development and document this in a professional portfolio as required by Health and Care Professions Council (HCPC).
To participate in the staff appraisal scheme as an appraisee and be responsible for complying with the agreed personal development plan.
To be responsible for equipment used in carrying out physiotherapy duties, and to adhere to departmental policy, including competence to use equipment and to ensure the safe use of equipment by others through, teaching, training and supervision of practice.
To be flexible to the needs of the physiotherapy service and willing to perform any other duties as required, following negotiation between manager and employee
