Job description

A rare opportunity has arisen in the Bournemouth Older Persons CMHT for someone who is motivated, enthusiastic and committed to working in a recovery focussed approach. The team works with a diverse group of services users who experience challenging mental health issues and alongside social care colleagues in providing a person focussed manner.

To be personally responsible and professionally accountable for a caseload of older adults in the community with severe mental illness who have multiple and complex needs.

Work alongside colleagues as part of a cohesive team in assessing people in community settings, delivering care plans, undertaking risk assessments and to provide high quality care and treatment in conjunction with other professionals through the Trust’s agreed multi-disciplinary assessment and treatment/care programmes.
