We have an exciting opportunity for an Integrated Community Team Lead to join the DCHS Wound Clinic Service management team.
This full time post will have management and leadership responsibility for the Derby City Wound Clinic Team, a 7-day outpatient service with a team consisting of 35 nurses and healthcare assistants.
This is a challenging, but rewarding role, where you will be required to use your clinical, operational, managerial, and leadership expertise. Whilst the post holder will be expected to work independently with a high degree of autonomy, they should be a team player who is able to work collaboratively with the wider management team and organisation.
As well as the day-to-day management of the team, the post holder will have a focus on achieving high quality care, improving services, and meeting performance objectives, whilst supporting the wider operational and strategic agender of the Wound Clinic Service throughout Derbyshire.
Whilst this role will initially involve managing the Derby City locality, there is an expectation that over time your portfolio will grow to include additional Wound Clinic Teams within the of county Derbyshire.
This role requires extensive travel throughout Derby City and Derbyshire in support of all eight Wound Clinic Teams.
You must demonstrate excellent interpersonal and communication skills and have the ability to manage and prioritise workload whilst being accountable for the operational management and leadership of the Derby City Wound Clinic Team.
You will need to engage effectively with your team, peer group, and external/internal stakeholders to support you in delivering your key performance measures.
This will require an inclusive, transparent, open approach to your role, where you are happy to be accountable to others and promote the concept of a leader who is challenging, whilst expecting to be challenged.
You will need an appetite to meet objectives, deliver change, develop and embed partnership working, maintain quality nursing standards, and drive service improvement and care for the benefit of the population.

The Wound Clinic Service is a high demand service where patient journey, experience, and clinical outcomes are prioritised, therefore you must be committed to ensuring quality care and sufficient patient flow.
A key part of this role is leading and supporting the team to ensure they deliver the best possible care whilst supporting them to develop professionally.
Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.
We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people’s own homes and via virtual consultations.
The DCHS Wound Clinic Service was established in April 2019 and delivers 65,000-70,000 outpatient face-to-face contacts per year.
The services is composed of eight Wound Clinic Teams (High Peak, Derbyshire Dales, Chesterfield, Northeast Derbyshire, Amber Valley, Erewash, Derby City, and South Derbyshire) and a dedicated call centre.
The DCHS Wound Clinic Service provides evidence-based wound care for hard-to-heal, chronic, and complex wounds.

As a member of the Wound Clinic Service management team, you will be required to provide support to all eight Wound Clinic Teams across Derbyshire as required including cross-cover with other Integrated Community Team Leads.
You must have NMC registration and be educated to degree level or have an equivalent clinical qualification. You must have clinical experience and a developed knowledge of community-based healthcare.
Operational management experience at an outpatient ward or team level is essential.
