Job description

We are seeking an enthusiastic and motivated Healthcare Assistant to join our team based at Ilkeston Community Hospital in the Erewash locality.

You will be required to travel within the Erewash locaility, as part of a wider integrated team providing holistic care to patients in their own homes and care home settings.

You will have experience working in either a health or social care setting.

A good standard of English is required, along with excellent communication skills.

As a clinical employee it will be your responsibility to ensure that your behaviour and values reflect those set out in the DCHS Way.

This post will involve working within a community nursing team visiting patients within their own homes.

To assess, implement and evaluate individualised patient care in the most efficient and clinically effective way whilst maintaining the patient’s privacy and dignity.

To provide the delivery of high-quality care and actively support the District Nurse in the management of the health care environment.

To work in close collaboration with the Integrated Community Teams.

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people’s own homes and via virtual consultations.

During the pandemic response we have led the implementation of public vaccination centres in collaboration with partners across health and social care.

  • Provide holistic care to patients in their own homes or alternative health care setting, under the supervision of the appropriate registered professional.
  • Communicate information to patients and carers regarding their health needs and associated treatment plans.
  • Report and take appropriate action regarding any change in patient condition or circumstance, and communicate verbal and written information to all relevant integrated care team members.
  • Maintain and record accurate health records in line with DCHS policies and procedures and Caldicott requirements.
  • Observe personal duty of care when using or supplying equipment, to ensure compliance with health and safety regulation and to monitor and report to appropriate registered clinician on the effectiveness of equipment and aids in patients home to maximize use of resources.
  • Ensure “Every Contact Counts” and contribute to public health and health promotion initiatives.
  • Under appropriate supervision, assist in routine screening for chronic disease management and record results for monitoring purposes to ensure compliance with NSFs, NICE etc.
  • Contribute to and support the maintaining of service standards and performance through audit and benchmarking, as per internal audit priorities, CQUIN, CQC Standards for Compliance etc; and to encourage colleagues and team members to do also.
  • Participate, as an individual and as team member, in continuous learning and reflective practice in order to maintain own knowledge and skills and share good practice with colleagues to continually improve services.
  • Actively contribute to protecting people at risk by assessing and monitoring the care environment and individuals and by following and acting on and within Safeguarding policies and procedures.
