We are seeking to recruit an enthusiastic and highly motivated community care worker to join our well established intermediate care tier in Warrington.
Our intermediate care services are made up of a range of services including:
Rapid Response Services in addition to our well established Care Call services we are also one of seven nationally acclaimed accelerator sites for NHS England’s Aging Well programme, delivering a new urgent community response service to people experiencing a health or social care crisis. Our highly skilled team is made up of health and social care professionals and carers working together to support people at home or in the community and in doing so contributing the avoidance of a hospital admission or an admission to respite care.
Intermediate Care at Home Services where we provide a wide range of care, social work, rehabilitation, therapy, equipment and assistive technology services supporting people to live well and independently either at home or in the community. We take pride in our GOOD CQC registration for our care services and our re-ablement outcomes are strong with over 80% of people receiving our service remaining independent at home for three months after leaving our service.
Hospital Discharge Team and Intermediate Care Bed Bases where we support the safe and timely transfer from hospital into settings that best meet individual needs, providing care and rehabilitation to regain independence and undertake assessments for their long term needs.
Working as a member of a forward thinking, supportive and ‘can do’ team, as an individual you will have care management skills and hold a current registration with Social Work England, multi-disciplinary work experience, excellent interpersonal skills, able to work in genuine collaboration with others, a strong person centred value base.
Intermediate Care offers short term therapeutic and social care assessment and support to adults within their own homes and within our intermediate care bed base service. The service prevents inappropriate hospital admissions, facilitates safe discharges from hospital and community bed based. The main focus is to maximise independence and prevent premature long-term care needs.
The Community Care Worker will work across the service assessing needs, risks and eligibility within the Care Act 2014, managing a caseload of service users who have been referred to the service. The role will involve working alongside therapists, service users, their carer’s and external agencies to complete strength based, person centred assessments of service users’ needs and risks during their time within the service and ensuring a seamless service for those with ongoing needs.
