Job description

Directorate: People Services
Hours: 37 hours a week
Contract Type: Permanent
Location: Council House

Derby City Council is looking to recruit a Community Care Worker on a fulltime permanent basis. You will deliver the very best service and outcomes to adults within the Learning Disability Assessment Team. As a team we support individuals with learning disabilities who may currently be living in a residential/nursing care home, supported living, independent hospital or with their family. You will be assessing and reviewing customer support packages with a view to promoting choice and control using person centered approaches.

You will also be supporting some individuals to move from residential care to a life in the community providing advice and guidance.

We wish to recruit an energetic and motivated individual, with good communication skills who can work effectively with other agencies. As a Community Care Worker, you will complete assessments and support plans that are person centered. It is desirable that you have some knowledge and experience in supporting individuals who have a learning disability. You will need to be able to prioritise your work and respond to and support customers changing needs, whilst demonstrating empathy and adapting your communication to the individual’s needs. Applications are welcome from candidates with the ability to encourage, support and engage with carers and vulnerable adults.

We are a friendly and dynamic team where the principle of supporting each other is held as good practice.

For further details please contact Lisa Beris (Senior Practitioner Tel: 01332-640376) or John Dhamrait (Team Manager Tel: 01332-640416).

As part of our safer recruitment checks the successful candidate will be subject to an enhanced DBS check.

All employees will normally be appointed to the minimum of the grade for the job. For reasons connected with a particular need of the Council it may be appropriate, subject to approval, to appoint to a higher point in the pay grade.

Before you complete your application form please read our guidance on filling in your application form.

Quick Tip! Remember to align your personal statement to the Essential Criteria in the Person Specification

Please be aware that where there are staff within the organisation who are 'at risk' of redundancy, requirements to redeploy to another suitable role will apply and take priority.

The closing date for this vacancy is 29 September 2023 at midnight. Any applications received after the closing date will not be accepted.
