We are looking for a committed and enthusiastic Community Care Worker to join a dedicated team, working to support Adult Social Care customers to maximise their independence in the community.
Our customers are aged 18 and over and may have support needs relating to their larning disability, physical disability, older age and mental health.
As part of your role you will work to assess customers’ needs and ensure they are provided with advice and when necessary support from Adult Social care to meet their needs.
The successful candidate will
- Demonstrate excellent communication skills and the ability to relate well with our customer and their carers, in addition to colleagues and other professionals including GP’s, Local Area Coordinators, Emergency Services and Care Agencies.
- Have the ability to work positively, in a person-centred manner with individuals to support them to achieve their potential and maximise their independence.
- Complete assessments of need under the care act, in line with the principles of strengths based practice, whilst considering responsible spending of public monies.
- Identify and address presenting risk to vulnerable customers.
- Have the skills to build positive links with community resources that can provide support to the customers.
- Be able to work independently but will also form part of a rota to deliver a city wide duty system.
- Prioritise work, be responsive to presenting issues and on occasion work to tight deadlines.
- Have good IT skills
- Demonstrate a reflective approach to their work and with support, learn from their work experiences to develop a level of resilience necessary to undertake the role.

All employees will normally be appointed to the minimum of the grade for the job. For reasons connected with a particular need of the Council it may be appropriate, subject to approval, to appoint to a higher point in the pay grade.
