An exciting opportunity has arisen for an experienced and motivated registered professional to join the North Locality CHC & Placements Team. We are looking for an experienced individual who can demonstrate excellent assessment and analytical skills, with experience in NHS Continuing Healthcare whose primary focus will be to assist in the delivery of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care (revised July 2022). You will support the Operational Manager to deliver on all the areas of CHC, FNC and Personal Health Budgets therefore previous knowledge and experience in CHC/FNC is essential.

- Supporting the Operational Manager to establish and maintain robust systems to implement current policy and guidance relating to the NHS Continuing Healthcare processes.
- Preparing good quality needs portrayals and assessments based on the National Framework Decision Support Tool.
- The line management, supervision & support, of the Specialist Practitioners within the team.
- Supporting the implementation of personal health budget agenda to our cohort of clients.
- Supporting Adult Safeguarding processes, ensuring that care packages and commissioned services are safe.
- To facilitate implementation of safe, effective and sustainable packages of care for individuals with complex needs and monitor and improve standards of care through clinical review.
