Job description
This role will primarily support the clinical management of patients on the 'Home Based Care' (HBC) pathway for patients with Parkinson's Disease at University Hospitals Plymouth NHS Trust. The HBC pathway is a unique and innovative Patient Initiated Follow-Up (PIFU) pathway that currently cares for approximately 250 patients, and uses remote monitoring and patient self-reported symptoms and a primary means of providing advice and treatment for People With Parkinson's (PWP). The Home Based Care pathway is also expanding into innovative digital technology which will enhance the levels of care and support for patients, and will support the pathway to provide timely and efficient interventions for patients when they are needed, as well as a wealth of resource and self-management material. This post-holder will provide an active role in the development and creation of the new digital pathway, and will help to shape the design of a new, digital outpatient model that will increase the clinical capacity for new patients, and may pave the way for other NHS organisations to provide specialist Parkinson's care for the growing number of People With Parkinson's in the UK.
