Medicines Management Acute Medical Admissions Optimises patients own medicines brought into hospital through direct or remote discussion with patients/family relatives/carersUses interpersonal skills to communicate the medicines optimisation strategy to the multidisciplinary teamUses interpersonal skills to ensure patients/relatives/carers understand the advantages of the Trusts Medicines Management Programme. This will require the use of excellent communication, good persuasive skills and knowledge of how to overcome barriers to understandingGains consent for the patients Summary care Record to be viewed, ensuring full compliance with access regulations and patient confidentiality proceduresRecords information relating to the patients medication onto the prescription sheet or in the future the epma system Following appropriate training and once deemed competent to do so, participate in the medicines reconciliation service at ward level. This will involve ordering medication, assessing patients own medicines and assessing the requirements for compliance aids. Provides pharmacy related information to the multidisciplinary team Answers basic pharmacy related queries from patients/relatives/carers, these can be routine or non-routine Problem solves with competency and refers more complex problems to senior staff as appropriate Communicates directly with community staff as necessary regarding patients medication Directly support nursing staff on medication ward rounds e.g.
assist in obtaining missing items so the patient does not miss a dose In conjunction with the ward based pharmacy teams, monitors medication omissions at ward level. Where a drug is unavailable at ward level, facilitate its supply through the pharmacist and/or pharmacy technician Facilitates in drug distribution systems ensuring inpatient medication arrives promptly and securely to the ward Investigates out of stocks at ward level and communicates the finding to the appropriate managers Ensures patients own drugs are transported safely and securely to the admitting ward Participates in medicine related housekeeper duties at ward level, this will include daily checks to ensure safe and secure storage of medication Completes medicine management checks as required at ward level Provides a put away service for medication supplied by the Pharmacy department Highlights medication issues communicated by the patients/relatives/carers to the appropriate member of the multidisciplinary team Ensures individually labelled medication is transferred with the patient following a ward transfer, communicating the transfer of medications with other pharmacy ward teams throughout the hospital Monitor ward pharmacy returns units returning reusable medication into pharmacy stock and processing waste medication accordingly Accurately label and dispense prescriptions for wards, departments and clinical areas within the Trust, when required Has knowledge of and follows all pharmacy and ward procedures Holds a mobile phone to respond to urgent supply issues and queries Evaluates own workload and prioritises as necessary Participates in audit and project work to develop the medicines management service Raises awareness of the Pharmacy team and the services provided through face to face contact with patients/relatives/carers Dispensing of Medicines To label and dispense prescriptions for in-patients and outpatients under the supervision of a Pharmacist in accordance with local and national policies and any Standard Operating Procedure (SOP) in place To receive prescriptions for patients and carers and verify patients details To input prescription data onto the pharmacy computerised record system Enter patients details on the pharmacy system from PAS The preparation of pre-packs and over-labelling of medicines The preparation of worksheets and labels for pre-packing and overlabelling Where medicines are unavailable arrange an appropriate follow-up supply and ensure these items are sent to patients where necessary To refer any prescription queries to the dispensary manager or supervising pharmacist Give out prescriptions to patients and carers, referring to a pharmacist or technician if information or education is required To ensure that the dispensary and dispensing stations are appropriately stocked To carry out reception duties including answering of the telephone from staff and patients To undertake the daily rolling stock take and check expiry dates of all medicines held; and advise the purchasing manager of those that are going out of date To file prescriptions, pharmacy orders and other documents as required To supervise other pharmacy assistants as designated by the dispensary manager and / or chief or principal pharmacy technician, following Standard Operating Procedures To participate in effective stock control and management systems for the various areas within the pharmacy To ensure that operational / technical audits are completed within the pharmacy department To participate in clinical audit as part of the multidisciplinary audit process To participate in the filling of emergency drug boxes for adults and paediatrics To implement procedures and deliver in-service training, as required, of other pharmacy assistants within the section To mentor less experienced and trainee dispensing pharmacy assistants. To contribute to the induction , training and mentoring of staff allocated to the department, including pre-registration students and summer students To contribute to the smooth and efficient running of the department, including cover of other staff To maintain safe systems of work at all times, including the knowledge of and abiding by the fire precautions, departmental and Trust safety policies and procedure manuals To promote the professional image of pharmacy within the Trust by ensuring that the standards of dress and behaviour reflects the professional service offered by the department To participate in any training, validation work or competency testing in line with the departmental requirements To participate in the Trust appraisal scheme and to agree training needs in line with continuing professional development To maintain a high level of professional practice To observe the regulations in respect of Health and Safety and Safety at Work Act, COSHH, Data protection Act, abiding by the fire precautions and departmental and Trust safety policies To ensure that safe working procedures are maintained and followed
