This is a really exciting time to join Homerton Pharmacy as we re-design our clinical services; enhance the role of prescribing pharmacists, support virtual wards and build an exemplar service fit for the future.
The post holder will be a key member of the care of the elderly and rehabilitation team, delivering high quality medicines optimisation service to patients, whilst developing effective working relationships within the multidisciplinary setting. They will be the lead for the care of the elderly and rehab wards. You will be liaising with the relevant teams including consultants & ward managers.
The post holder will be involved in the training and development of rotational pharmacists and Medicines Management Technicians within the team.
It is expected that the successful applicant will be an experienced clinical pharmacist with substantial experience in providing clinical pharmacy services to acute medicines and who has successfully completed a Diploma in Clinical Pharmacy, is an independent prescriber or working towards this
