To support the development of clinical protocols and management plans for patients referred to the service. Support the development of admission criteria, clinical protocols and care pathways agreeing performance measures relating to discharge. Length of stay and patient outcomes and leading on the audit and research of nurse led care. Develop integrated care pathways in collaboration with other relevant professionals, specifically interfacing with the long-term conditions, case management and end of life agenda to benefit the delivery of the clinical service.
