Please See Supporting Information For Full Job Description. Job responsibilities: 1. Clinical1.1. To work as an autonomous practitioner to assess, monitor and evaluate treatment efficacy and management of the patient pathway.1.2.
To administer and monitor medication according to Patient Group Directions or as an Independent/Supplementary Nurse Prescriber with correct qualifications, training and support1.3. Demonstrates specialist knowledge and shares this with others to positively influence patient care.1.4. Improve and maintain standards of care with the speciality.1.5. Provides specialist advice and education for patients and relatives/carers.1.6.
Develops education programmes for patients/carers to promote their understanding and evolve strategies for self-management.1.7. Acts as a specialist advisor in all matters relating to paediatric dermatology to health care workers within the Trust and provide support, where specific medical advice is not required.1.8. Communicate very sensitive condition related information to patients, family and carers, requiring high levels of empathy and reassure, using advanced communication skills 1.10. Acts as patients advocate where relevant, 1.11.
Acts as a point of contact for the patients and their families for advice and support. 1.12 To accept referrals for new patients in the Paediatric Eczema and Acne clinics and agree a diagnosis, devise a treatment regime and plan future review/discharge as appropriate. To provide nurse led follow up clinics including Acne, Eczema clinics, second line monitoring and telephone clinics. 1.13 To input into Paediatric dermatology ward patients, by providing support to the dermatology consultants, paediatric ward staff (on application of treatments/care plan) and parents during admission.
1.14 Child protection concerns: - discuss with consultant, Lead Nurse and follow Child Protection Procedures/protocols 1.15 To be aware of and undertake responsibilities with regard to Child Protection procedures (both national and local), liaising with the multidisciplinary team and working within the philosophy that the needs of the child are paramount. 2. Communication and Team Working2.1. Establish effective working relationships between the Dermatology service, the Paediatric service and other departments within the Trust, to promote the dissemination of information, knowledge and good practice.2.2.
Communicate effectively with patients, family and carers to assess their needs and provide/facilitate relevant support information education advice and counselling where necessary.2.3. Demonstrate advocacy and mediation skills when communicating between patients, carers and professionals. 3. Administration 3.1.
Ensure accurate documentation of patient interactions are maintained. 3.2. Ensure statistical information is collated on new patient referrals and accrued workloads. 3.3.
Participates in the development of relevant patient & professional documentation within the clinical speciality. 4. Managing People and Resources 4.1. Prioritise own workload and manages time effectively, when working alone or in collaboration with others.
4.2. Operates autonomously at an advanced level of clinical practice to provide research based expert paediatric dermatological care and advice to all colleagues. 4.3. Demonstrates professional leadership behaviours acting as a positive role model for other staff.
4.4. Identifies risk within the sphere of responsibility and plans, implements and evaluates strategies to reduce and manage the risk. 4.5. Ensures that the Dermatology Lead Nurse is informed of any concerns.
4.6. Identifies and utilises adequate professional and lay resources or facilities at local and national level. 5. Education and Training 5.1.
Formally and informally teaches patients and relatives. 5.2. Assesses training and education needs based on feedback from audit, identification of poor practice, complaints, national guidelines etc. 5.3.
Carries out formal/informal teaching on aspects of paediatric dermatology care to all staff groups within the Trust and Community as required. 5.4. Provides patient and carer education and health promotion information. 5.5.
Act as an education resource for all disciplines on paediatric dermatological issues during working hours. 6. Research and Audit 6.1. Undertakes audit and evaluation of practice within own area of practice.
6.2. Supports the implementation of research and audit findings. 6.3. Audits and assists with the revision of policies, guidelines and protocols related to the service.
7. Policy & Service Development 7.1. Identifies the need for service developments to improve clinical effectiveness and efficiency. 7.2.
Participates in the development and implementation of Trust-wide policies, clinical guidelines and protocols necessary to support the delivery of paediatric dermatologial care. 7.3. Participates in the departmental audit program and evaluation of practice. 7.4.
Leads on establishing, facilitating and evaluating nurse led initiatives. 8. Professional Conduct and Professional Development 8.1. Develops and maintains own professional knowledge and skills through life long learning.
8.2. Uses reflection to identify and prioritise areas for self-development. 8.3. Exercises professional accountability and adheres to the NMC Code of Professional Conduct.
8.4. To keep abreast of developments in paediatric dermatology care by attending relevant study days and courses disseminating this information to all groups of staff. 8.5. Develops and annually reviews, in conjunction with the Lead Dermatology Nurse, a personal professional development plan.
