Main duties of the job
This pilot will work closely with two GP practice pilot sites to establish evidence based effective approaches to improving Hep C care across four key patient groups:
- Patients that may be at risk of Hepatitis C
- Patients who have a confirmed diagnosis but no treatment record and need to be followed-up.
- Cases of those that may have received infected blood and/or infected blood products
- Patients with other blood borne viruses (BBVs)
It would suit any GP who is passionate about making a definitive improvement to patient outcomes whilst also diversifying their clinical role.
Proposed approach is as follows:
- Set out a 9-month pilot programme. First 2 months will be a scoping exercise to identify and work with the best pilot sites to capture the most activity. Run the pilot for 6 months and the final month, wrap up pilot and facilitate an after-action review and evaluation.
- Engage the pilot site staff and identify a core team in each who will work on the development of an effective service. The team will represent the delivery chain of staff involved in the patient pathways.
- Collaborate with the National Hep C team for Eliminating Hep C route map to ensure joined up approach and guaranteed value add for the pilot sites and patient outcomes and maximum patient impact.
- Work with the teams to agree aims and families of measures for each element of the project and collect data to monitor progress.
- Develop a tool kit to be shared with other sites for adoption.

As Clinical Lead for Hep C you will work as part of a dynamic team to support Transformation Partners in Health and Care (TPHC) to transform care for patients.
This will include to lead the planning and design of the strategy to meet the vision and strategic direction and to ensure outputs are delivered effectively and to time, and that all pan-London outputs align with, and support, local transformation programmes across Integrated Commissioning Boards (ICBs), Integrated Care Systems (ICSs) and Strategic Planning Groups (SPGs). Work across the wider organisation to agree prioritisation of blocks of work and related resource allocation to ensure high priority work/dependencies are completed in a timely manner. We will work with you to ensure that you are supported in developing your career whilst having the opportunity to work on some of the most strategic and transformative programmes in London.
