JOB TITLE: Clinical Coder

BASED Ash Trees Surgery but covering the work of all sites

REPORTS TO: Clinical Coding Manager – Catherine Lawrence

JOB SUMMARY: In this role you will be working as part of a team of dedicated, clinical coders who convert clinical information from patients’ medical records to classification codes. To work accurately and methodically, in a timely manner whilst paying attention to detail and demonstrating a good knowledge of medical terminology. Displaying excellent IT and organisational skills, is essential to this role as is the ability to remain professional under pressure and have the ability to be able to remain calm when faced with the more challenging parts of this role. Working effectively within the team, using your own initiative, whilst supporting and helping the wider practice teams is pivotal.

KEY DUTIES AND RESPONSIBILITIES

  • Analysing patient records and recording all the relevant information on Emis Web Healthcare System.
  • Opening post, both internal and external, and scanning all documents onto our document management system.
  • Ensuring that all clinical codes are appropriate in relation to letter detail and QOF (Quality and Outcomes Framework).
  • Collaborating with physicians to ensure information accuracy.
  • Summarising newly registered patients’ records. Ensuring all clinical information is documented correctly.
  • Dealing with electronic transfers of patient notes from other practices and rectification and re-coding of data.
  • Ensuring all emails are read and actioned as appropriate.
  • Use the clinical system to task and resolve coding queries and take appropriate instruction from clinicians.
  • Following up and requesting of outstanding clinic letters, results.
  • To be accessible for and provide support to other teams within the practice as required.

Personal Attributes

  • Competent with word, outlook & excel, Emis Web and Docman (desired but not essential).
  • Attention to detail a methodical approach and accuracy is of key importance.
  • Desire to learn and develop, willingness to attend training and online workshops.
  • Have excellent communication skills both written and verbal, with good spelling and use of grammar, typing skills must be of a good standard.
  • Have the ability to work under pressure and prioritise workload, calmly and confidently.
  • Demonstrate an excellent knowledge of medical terminology.
  • Be flexible and a team player supporting colleagues as required.
  • To be respectful and supportive of colleagues and those in the wider practice
  • Demonstrate the ability to work on your own initiative.

Health & safety:

The post-holder will assist in promoting and maintaining their own and others’ health, safety and security as defined in the practice Health & Safety policy, the practice Health & Safety manual, and the practice Infection Control policy and published procedures. This will include:

  • Using personal security systems within the workplace according to practice guidelines
  • Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks.
  • Making effective use of training to update knowledge and skills.
  • Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards.
  • Actively reporting of health and safety hazards and infection hazards immediately when recognised.
  • Keeping own work areas and general / patient areas generally clean, assisting in the maintenance of general standards of cleanliness consistent with the scope of the job holder’s role .
  • Undertaking periodic infection control training (minimum annually).
  • Reporting potential risks identified.

  • Discuss with other members of the team how the policies, standards and guidelines will affect own work.
  • Participate in audit where appropriate.

Salary: £20,103.00-£21,415.00 per year
