Job description
Under direction of the Theatre Admin Manager and the HDU Manager, the post holder will have the responsibility for maintaining the HDU clinical databases required for ICNARC (Intensive Care National Audit and Research Centre), CCMDS (Critical Care Minimum data set) and assist in providing reports and feedback to the Clinical leads. As well as other Ward Clerk activities as outlined below.
1. Ensure patients are registered onto the Patient Administration System (PAS, LORENZO) and admitted onto ICNARC and CCMDS.
2. Record patient’s admission, physiology and outcome details from the HDU charts and patients hospital notes for ICNARC & CCMDS data entry on Medicus.
3. Liaise with Medical staff to ensure accurate coding of inpatients episodes for ICNARC and CCMDS data entry, Collect relevant information from unit patient prescribing information communication system (PICS)
4. Liaise with medical records requesting notes collecting discharge information. Communicate with other hospitals and audit clerks to complete patient outcomes, either by phone calls or using the E –mail system.
5. Prompt completion of the ICNARC Data Validation Report.
6. To provide administrative support to the Senior Nurse and assist with ad hoc information requirements.
7. Provide monthly reports if requested by clinical leads or MDT in support of research projects or Data review.

1. o analyse data collection and escalate any areas of concern to the Clinical leads.
2. Provide training and awareness sessions to new members of staff so that the service can continue in your absence.
3. Attend regular update sessions to develop and learn new requirements of ICNARC/CCMDS data collection
4. Show an understanding of medical terminology, anatomical and Physiological terms. Have a basic Knowledge of orthopaedic procedures.
5. Compile a monthly CCMDS/Lorenzo discrepancy list and complete any missing data, ensuring both systems have accuracy and send to informatics for finalisation.
6. To draft, type correspondence and compiling letters and memorandum and using own initiative.
7. Following directions from the Senior Nurse and audit Lead Consultant undertake audits and collate results.
8. To provide high quality reception duties through welcoming and greeting patients, relatives, staff and visitors to the ward in a professional and courteous manner; dealing appropriately with enquiries and ensuring each is followed up and responded to, thereby participating in good public relations between patients, public and staff.
9. To organise the repair of clinical equipment/services on the ward by liaising with Estates/Facilities, IT help desk, switchboard to facilitate its prompt return.
10. To liaise with Medical Records and other departments to ensure the availability of patient’s notes, collecting as required.
11. To ensure the accurate and timely update of the PAS/Lorenzo system and unit admission book, to include details of admissions, transfers, discharges and any changes to patient details.
12. To be familiar with the Trust’s policies e.g. Infection control, Uniform, and no smoking.
13. Take responsibility for the collection and disposal of confidential waste.
14. Undertake general admin support duties for the unit: photocopying, distributing post, laminating etc.
15. Answer the telephone promptly, politely and accurately stating location, designation and name. To take incoming telephone calls and e-mails ensuring they are dealt with efficiently and effectively at all times. Ensure accurate message taking and channel information/requests to appropriate members of the multidisciplinary team.
16. Ensure effective two way communication, as appropriate with visitors and staff in accordance with Trust policy.
17. Be conversant with and adhere to the Data Protection Act and protect the confidentiality of patients, relatives and staff within the guidelines (Ref Caldicott)
18. Be mindful of potentially confrontational situations and where possible attempt to defuse the situation or seek help and advice from senior staff (Ref violence an aggression)
19. To undertake any other duties as may be required from time to time which are commensurate with the nature and grading of the post
This role requires limited physical effort; however there may be the requirement to travel between clinical and non-clinical work locations during the course of the working day and some lifting or carrying of notes.
The post holder will undertake other duties as may be required to achieve the Trust’s objectives, commensurate with the grading of the post.
