Job description
This role is full time for 6 months initially, to start ASAP and paying a daily rate of £320 Umbrella inside IR35. Staff will also be required to attend the office a maximum of 2 days per week.
Job Purpose:
- The post holder will be responsible for the assessment of adults including the community/nursing homes and acute care to determine eligibility to receive Fully Funded NHS Continuing Care or Funded Nursing Care [revised November 2018].
- The role will cover the following care groups:
o Learning Disability
o Physically and or Sensory Disabled
o Physically frail
o Palliative Care
- The post holder will be the Coordinator of the multidisciplinary team meeting whose purpose is to establish if an individual has a Primary Health Need and eligible to receive NHS Continuing Healthcare. The post holder will undertake and ensure that the multidisciplinary team produce high quality holistic assessments and re-assessments/reviews in a variety of settings e.g. acute hospital, nursing home and community to ensure that the application of the NHS National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care process is robust and compliant with all guidance and best practice. The post holder is responsible for providing clinical support to the commissioning of a suitable care package to meet the individuals care needs.
- Provide accurate advice and information to clients, families and colleagues, regarding the Continuing Healthcare process and the MDT recommendation throughout the assessment/review process.
Duties and Accountabilities:
- Using national and local assessment tools, adhering to the lead times, coordinate/lead/ participate and support the assessment process by the multidisciplinary team to determine the eligibility as set out in the National Framework for Continuing Care explaining to patients, families, health and social care colleagues and care providers, so that the process is transparent and clearly understood
- To undertake assessments and reviews, maintaining appropriate contemporaneous written records and communicate information as necessary with all relevant parties whilst maintaining appropriate confidentiality of the patient record.
- Identify and support the MDT in obtaining further specialist evidence to support the assessment process
- Daily recording on data systems to ensure contemporaneous record keeping from a patient and financial perspective to maintain an effective audit trail
- Working across health and social care boundaries to provide leadership and advice to educate, inform and involve clients' carers and professionals on the National Framework for Continuing Health Care guidance and matters of local service provision.
- Provide clinical advice to the commissioning process of care packages ensuring a co-ordinated approach to care, reflecting individual's preferences, and taking into account safe delivery of care.
- Reporting to providers and escalating concerns or quality issues in accordance with SVA and/or the delivery of commissioned care to line manager
- To act as patient advocate to support vulnerable adults and where necessary identify and report any situations of adult abuse immediately to their line manager and commissioners
Education/Knowledge and Qualifications:
- Registered Nurse Qualification
- Evidence of professional development
- Relevant post basic qualifications/training
- Demonstrable relative experience and knowledge of current key policies and priorities
- Demonstrate knowledge and understanding of The National Framework for Fully Funded NHS Continuing Care
